Listed below are the CDT-4 procedure codes adopted by the DCH for dental services. Any new CDT-4 procedure code not listed will not be covered at this time. All procedure codes being deleted from the national code set will be eliminated from use for dates of service on. The attached Dental Procedure Codes Appendix lists the covered dental procedure codes. This bulletin also transmits the revised Chapter III (Coverages and Limitations). As a part of the chapter revision process, previously issued policy bulletins have been incorporated.
